We welcome you to our office...

This page is designed to help you make your new patient experience with our office as smooth as possible.

In our efforts to go green, we have become chartless and make every effort to minimize paper use.  Part of this process requires the use of technology and this website.  Of course we accommodate you with paper office forms if necessary.

Your first step as a new patient is to call the office and set up a new patient appointment.  Our front office staff will assess your needs and make you an appointment date and time for a comprehensive initial examination.

We do not need copies of your old dental records, but please request copies of your last dental radiographs, especially any complete sets, and bring them to your new patient appointment.  Another option if your previous dentist uses digital radiography is to have them e-mail a copy of them to our office.

The last thing you need to do before your new patient appointment is to fill out our new patient forms.  You can complete these forms online through the website.  The link is to the left, "New Patient Forms". 

  1. Consent for Internet Communications (1 page)
  2. Adult New Patient Form & Medical History ( 12 computer pages)
  3. Child New Patient Form & Medical History (5 computer pages)

When you click on the link "New Patient Forms", it will open a small window that will request the patient's name AND appointment date for your new patient appointment.  If you are a parent registering for your child, please be sure to enter your childs name and fill out forms for each child separately (if there are more than one).

Click on the form you want to fill out, use the blue small arrow buttons to scroll to to the next page, and press submit when you are finished with all the pages in each form.

When you arrive for your new patient appointment, we will review your forms, and you will be able to digitally sign them.
